Your First Visit
At your first visit to Dr McClurg’s Smile Center you will be meeting with our New Patient Coordinator for a 45 minute appointment. This initial appointment will consist of a thorough examination which will provide us insight into your orthodontic needs. After the assessment, our new patient coordinator will review your needs and give you potential treatment options.
During the New Patient Consultation we will address the following questions that cover the fundamentals of your Orthodontic treatment. We encourage you to ask questions you may have as well.
- Is there a condition that Orthodontic treatment can help?
- Is Orthodontic treatment beneficial now or would it be more beneficial to be postponed for appropriate growth, tooth eruption or other factors have occurred?
- What different treatment options do I have?
- Do I need extractions?
- How long will I be in treatment?
- What is my total out of pocket cost?
- How much will my Insurance cover of my Orthodontic treatment?
- What payment options are available?
- New Patient Heath History Form
- Signed Acknowledgement on Getting Started at Dr. McClurg's Form
- Signed HIPPA consent
- If you have any Orthodontic insurance, bring your insurance card and photo ID. By providing this information at the first visit, we will be able to verify your benefits and provide you a more accurate estimate of your out of pocket costs.
- Any panoramic X-ray taken within the past six months.
The New Patient Consultation is very informative; when you decide to start treatment with our office the following appointment (records) is another appointment that is very educational. At the records appointment you will meet with the Doctor and confirm your treatment needs. Dr. McClurg’s Smile Center believes it is important to evaluate each patient’s specific needs, because in-depth planning leads to superior results and higher patient satisfaction.
